Physical Activity Levels and Associated Factors Among Persons with Type 2 Diabetes Mellitus: A Community-Based Cross-Sectional Study from Urban North–East Delhi, India

Introduction: India, the ‘Diabetes Capital of the World,’ is facing a rapid rise in diabetes cases, projected to nearly double by 2050. Physical activity plays a critical role in diabetes prevention and management, yet adherence remains low. This study examines physical activity levels and associated factors among individuals with type 2 diabetes mellitus (T2DM) in an urban slum setting of North–East Delhi. Methods: A community-based cross-sectional analytical design was employed, involving 301 adult participants selected through simple random sampling. Data were collected using the validated International Physical Activity Questionnaire (IPAQ-SF), along with sociodemographic information, anthropometric measurements, and glycaemic assessment. Statistical analysis included chi-square tests and multivariable logistic regression to determine independent predictors of physical activity. Results: Physical inactivity was prevalent in 65.4% of participants. In multivariable analysis, age ≥ 60 years (aOR = 1.89) and formal education (aOR = 1.89) were independent positive predictors of sufficient physical activity, while uncontrolled random blood sugar (≥200 mg/dL) was a negative predictor (aOR = 0.50). Physical activity showed a significant inverse correlation with blood glucose levels (ρ = −0.219, p < 0.001). Conclusions: Physical inactivity was prevalent among individuals with T2DM in this urban slum setting. Age, education, and glycaemic control were independent determinants of physical activity among urban individuals with T2DM. Walking was the predominant activity, and a significant advice–practice gap was observed. Primary healthcare interventions targeting these determinants are needed.
