From genotype to circuitry: closing the loop in diffuse midline glioma with proteomics, metabolomics, and AI
H3K27M altered diffuse midline glioma (DMG) remains incurable, but the first accelerated drug approval, granted on response rate with overall survival benefit not yet established, and the first volumetric regressions after CAR T cell therapy show that the disease is not unresponsive. The next gains will come from describing DMG at the level at which drugs act: proteins, their post translational modifications, and the metabolites that sustain them. I propose methyl signaling as a testable circuit connecting chromatin, metabolism, and the proteome, with an explicit prediction of antagonism between two of its nodes; I set out what a virtual DMG cell can and cannot be built from today; and I argue that liquid biopsy, locoregional delivery, adaptive trials, and federated sharing are the elements that close the loop between discovery and treatment.
