Dual Remission of Atopic Dermatitis and Frequently Relapsing Nephrotic Syndrome With Dupilumab

Nephrotic syndrome in children and atopic dermatitis (AD) share a common type 2 inflammatory substrate driven by interleukin (IL)-4 and IL-13 dysregulation. We report the case of a 9-year-old girl with AD and frequently relapsing steroid-sensitive nephrotic syndrome. Given the broad atopic phenotype and increasing relapse frequency despite dual immunosuppressive therapy, dupilumab 300 mg every 2 weeks was introduced. The patient achieved rapid and complete clearance of cutaneous lesions alongside sustained complete remission of nephrotic syndrome, maintained over nearly 2 years. Both corticosteroids and tacrolimus were fully discontinued within 2 months of dupilumab initiation, with no subsequent relapse of either condition. This case supports the hypothesis that dupilumab may represent an effective steroid-sparing therapeutic option in children with nephrotic syndrome arising in an atopic context.

Abstract

Nephrotic syndrome in children and atopic dermatitis (AD) share a common type 2 inflammatory substrate driven by interleukin (IL)-4 and IL-13 dysregulation. We report the case of a 9-year-old girl with AD and frequently relapsing steroid-sensitive nephrotic syndrome.
