Analysis code: hypertension care cascade and glycaemic state (CHARLS, HRS, ELSA)

Analysis code for "The glycaemic gradient in cardiovascular risk is present at blood-pressure target and attenuated above it: 18,434 adults in three observational cohorts". Contains Code S1-S14 as itemised in CODE_MANIFEST.md: cohort assembly, exposure construction, pooled Cox models, absolute-risk tables, figures, supplementary tables, and the diagnostic analyses (proportional hazards, additive interaction, treatment-based definition, sex-specific models, age-banded detection yield, and the health-behaviour sensitivity analysis). No participant data are included; CHARLS, HRS and ELSA are available from their respective repositories.
