Physician Leadership in Skilled Nursing and Assisted Living Facilities: Integrating Clinical, Organizational, and Stewardship Roles in Population Health and Accountable Care
Skilled nursing facilities (SNFs) and assisted living facilities (ALFs) have become central to population health strategies and accountable care organization (ACO) models as health systems assume accountability for outcomes and total cost of care. Residents in these settings are medically complex and require coordinated management across care transitions. Many physicians are insufficiently prepared for the organizational, regulatory, and relational demands of these roles, particularly within value-based and ACO-aligned care models. This discussion article defines core leadership domains and competencies required for effective physician leadership in SNF and ALF settings within a population health and ACO framework. Physician leaders in SNF and ALF settings function as integrators, aligning bedside care, regulatory accountability, operational feasibility, and value-based performance goals within ACO and population health strategies. Intentional preparation and development of physicians for these roles are essential to reducing avoidable utilization, improving quality outcomes, and sustaining high-value care across the continuum.
