Comparison of Serum Creatinine Levels in Controlled and Uncontrolled Diabetes Mellitus Patients Based on HbA1c Levels at Hermina Arcamanik Hospital

Diabetes mellitus can lead to renal complications as a consequence of poor glycemic control. HbA1c is used to assess glycemic control, whereas serum creatinine can assist in evaluating kidney function. This study aimed to determine differences in serum creatinine levels between patients with controlled and uncontrolled diabetes mellitus based on HbA1c levels at Hermina Arcamanik General Hospital, Bandung. A quantitative comparative design with a cross-sectional approach was employed. Data were obtained from the medical records of patients with type 2 diabetes mellitus, involving 100 samples from a population of 150 patients. Analysis was performed using the Shapiro–Wilk test and the Paired Samples t-test. The mean serum creatinine level was 0.681 mg/dL in the controlled group and 1.02 mg/dL in the uncontrolled group. The data were normally distributed, with p-values of 0.168 and 0.459, respectively. The Paired Samples t-test demonstrated a statistically significant difference (p<0.001), with a mean difference of 0.339 mg/dL. Thus, serum creatinine levels differed significantly between controlled and uncontrolled diabetes mellitus patients, with the uncontrolled group showing a higher mean value. Assessment of kidney function should also consider additional parameters, particularly estimated glomerular filtration rate (eGFR) and urine albumin-to-creatinine ratio (UACR).
