Care at home under healthcare 5.0: Toward a framework for resilient and patient-centric digital health transformation

Healthcare 5.0 is an emerging paradigm that builds on Industry 5.0 principles to enable intelligent, resilient, sustainable, and ethically grounded healthcare systems through advanced technologies and stakeholder integration. While existing studies have examined its technical components and policy implications, the field still lacks a consolidated framework explaining how Healthcare 5.0 can support digitally enabled care at home. This paper addresses this gap by developing a review-based conceptual architecture from a content-centric review of 107 academic sources. Using care at home as an illustrative lens, the study identifies the digital enablers, design principles, stakeholder roles, care delivery arrangements, and value structures needed to support decentralized, sustainable, and patient-centric healthcare. It shows how real-time data exchange, AI-enabled automation, interoperability, IoMT, digital platforms, and human-centered governance can integrate home settings into broader healthcare networks. The paper also discusses key socio-technical barriers to implementation and outlines implications for policy, healthcare organizations, technology design, and future research. By framing care at home as a strategic delivery layer of Healthcare 5.0, the study contributes a structured literature synthesis and review-based architecture for digital healthcare transformation.
