Ward Failure After Severe Rib Fracture: What Unplanned ICU Transfer Teaches Us About Early Risk Stratification

BackgroundSevere rib fractures are a major cause of morbidity and mortality after blunt thoracic trauma. The decision to directly admit these patients to the intensive care unit (ICU) vs initial ward placement remains debated. An unplanned ICU transfer may reflect delayed recognition of deterioration and worsen outcomes.MethodsAdult patients (≥18 years) with isolated severe thoracic injury (thorax AIS ≥3, AIS ≤1 in all other body regions) due to blunt trauma that resulted in one or more rib fractures were identified in the ACS Trauma Quality Improvement Program database from 2013-2021. Patients were stratified by admission pathway: direct ICU admission or unplanned ICU transfer from the ward. The primary outcome was in-hospital mortality. Multivariable Poisson regression with robust standard errors was employed to adjust for potential confounding.ResultsAmong 52,332 patients who sustained isolated severe thoracic injury with rib fractures and were admitted to the ICU, 1830 (3.5%) experienced an unplanned ICU transfer. Adjusted analyses confirmed increased risks of mortality (RR 3.35, 95% CI 2.89-3.88), pneumonia (RR 2.68, 95% CI 2.08-3.45), ARDS (RR 4.52, 95% CI 3.42-5.97), and mechanical ventilation (RR 3.21, 95% CI 2.98-3.47) with unplanned admission.ConclusionsUnplanned ICU admission is independently associated with markedly worse outcomes in isolated severe thoracic injury with rib fractures. These findings suggest that improved early risk stratification may help identify patients at increased risk of subsequent unplanned ICU transfer.
