LARA: The Leila Ahmadi Approach to Refractory Conditions A Three-Phase Conceptual Framework for Assessment, Preparation, and Individualised Treatment

Abstract Background: Conditions that respond inadequately to previous interventions may require a structured approach to assessment, treatment sequencing, and individualised management. However, the organisation of preliminary assessment, preparatory intervention, and subsequent condition-specific treatment within a single conceptual framework warrants explicit documentation. Objective: To describe LARA (The Leila Ahmadi Approach to Refractory Conditions), a conceptual framework developed by Leila Ahmadi for organising the assessment and individualised management of conditions with unsatisfactory responses to previous interventions. Framework Description: LARA comprises three sequential phases: (1) preliminary assessment of relevant clinical information and potential factors influencing treatment planning; (2) preparatory intervention addressing selected factors identified during assessment; and (3) individualised treatment guided by the presenting condition and clinical reasoning. The framework draws on existing therapeutic concepts and modalities, including photobiomodulation and auricular assessment, while organising selected elements within a named, staged approach. Its defining conceptual feature is the placement of a preparatory intervention phase between preliminary assessment and subsequent individualised treatment. Scope and Limitations: This document provides a conceptual description and authorship record rather than an empirical evaluation. It does not establish clinical efficacy, validate diagnostic methods, demonstrate a specific biological mechanism, or claim originality of the underlying therapeutic modalities. The originality of the overall sequence, clinical utility, reproducibility, and safety require further literature review and appropriately designed evaluation. Conclusion: LARA is presented as a three-phase conceptual framework intended to structure clinical reasoning and treatment sequencing in conditions with unsatisfactory responses to previous interventions. This publication establishes a dated, citable description of the framework as formulated by its developer and provides a foundation for subsequent methodological development and clinical evaluation. Keywords: LARA; conceptual framework; refractory conditions; treatment sequencing; individualised treatment; photobiomodulation; auricular assessment; clinical methodology.
