Implementing RN–Led Medicare Annual Wellness Visits in Rural Primary Care: A Mixed Methods Evaluation of the Geriatric Interprofessional Team Training in Primary Care Program
PURPOSE: Medicare annual wellness visit (AWV) uptake remains uneven, particularly in rural primary care. RN-led AWVs may support expanded preventive care capacity when implemented using structured strategies. METHOD: We conducted a mixed methods quality improvement evaluation of the Geriatric Interprofessional Team Training in Primary Care program across five rural primary care practices in northern New England, guided by Kirkpatrick's Four Levels of Learning. RESULTS: Of participants, >80% rated the training as relevant. RN knowledge and confidence increased following participation. RN involvement in AWV delivery increased over time and was sustained following the learning collaborative. Total AWV volume varied across project phases. CONCLUSION: Structured interprofessional training supported the adoption of RN-led AWVs and enhanced capacity for preventive care delivery in rural primary care.
