OPTIMIZED SURGICAL MANAGEMENT AND TREATMENT OF PERFORATED GASTRODUODENAL ULCERS

The authors report that contemporary studies have demonstrated that the issue of selecting the appropriate extent of surgical intervention for perforated gastroduodenal ulcers (PGDUs) remains unresolved, while the problem of surgical treatment continues to be of scientific and practical significance. This highlights the need to optimize surgical strategies and improve the comprehensive management of patients with PGDUs. The authors conclude that the implementation of an optimized surgical strategy in the main study group was characterized by a differentiated selection of the extent of surgical intervention, along with an increased use of minimally invasive and organ-preserving procedures The aim of the study. To improve the outcomes of surgical treatment in patients with perforated gastroduodenal ulcers by optimizing surgical management strategies. Material and methods of the study. The study was conducted at the clinical base of the Department of Surgical Diseases and the Andijan Branch of the Republican Scientific Center for Emergency Medical Care during the period from 2020 to 2025. Results. In the main group, during the period from 2023 to 2025, we developed an algorithm for selecting surgical management strategies and treatment methods for patients with perforated gastroduodenal ulcers (PGDU), which enabled the optimization of comprehensive surgical management. The following procedures were introduced: laparoscopic closure of the ulcer perforation, excision of the perforated duodenal ulcer with duodenoplasty, and a modified resection technique (Invention Patent No. IAP 8104, Intellectual Property Agency under the Ministry of Justice of the Republic of Uzbekistan, dated June 18, 2025).
