AI Uncovers the Final Line of Defense in DeFi Security: Technical Anatomy of the $128M Balancer ComposableStablePool Rounding Exploit (TOB-BALANCER-004), the Six Fatal Vulnerability Patterns of ERC-4337 Account Abstraction, and Bridging the Formal Ve

This monograph permanently notarizes and mathematically formalizes the author's eleven-sheet primary treatise, "AI Uncovers the Final Line of Defense in DeFi Security — Target Audience: Developers and Smart Contract Auditors" (「AIが暴く、DeFiセキュリティの最終防衛戦」対象:開発者・スマートコントラクト監査人, Sheets ①–⑪). First (Chapter 1, Sheets ①–④), we dissect the $128 million, 30-minute, 7-blockchain exploit of Balancer Protocol's ComposableStablePools. We prove that the root cause was neither a zero-day compiler flaw nor a private-key compromise, but a violation of DeFi's fundamental directional rounding invariant: using floor truncation (FixedPoint.mulDown) instead of ceiling rounding (mulUp) when upscaling exactOut swap amounts. While Trail of Bits had identified this exact behavior in October 2021 (TOB-BALANCER-004, severity "Undetermined") and September 2022 auditors excluded StableMath as "previously audited," nesting the pool's own liquidity token (BPT) inside ComposableStablePools allowed an attacker to flash-drain balances to 8–9 wei and execute 65+ batchSwap iterations to collapse the StableSwap invariant D. Second (Chapter 2, Sheets ④–⑧), we formalize the Six Fatal Vulnerability Patterns of ERC-4337 Account Abstraction identified across Trail of Bits audits—(2.1) unguarded execute, (2.2) gas-field omission in signature hashing (preVerificationGas, verificationGasLimit, callGasLimit, maxFeePerGas, maxPriorityFeePerGas), (2.3) stateful storage mutation during validateUserOp, (2.4) ERC-1271 cross-account/cross-chain replay lacking EIP-712 binding, (2.5) post-validation execution-revert gas griefing, and (2.6) EIP-7702 initialize(owner) front-running—alongside canonical Solidity fixes and EEA EthTrust Security Levels v3 [Q] compliance. Third (Chapter 3 & Closing, Sheets ⑧–⑪), we expose the fundamental limitation of Formal Verification (which only verifies human-written specifications, leaving implicit assumptions unguarded against offensive AI) and formalize the 2026 Three-Layer Defense Stack: (Layer 1) Formal Verification, (Layer 2) Trail of Bits' LLM-driven AI Dimensional Analysis (93% recall on implicit unit and rounding-direction bugs), and (Layer 3) Continuous Fuzzing (Echidna), anchored in our 64-bit Goldilocks prime field (Fp, p = 2^64 - 2^32 + 1) QFS-zkVM and sub-100-ns HIC hardware gating. License & Integrity Covenant: Creative Commons Attribution 4.0 International (CC BY 4.0) with Strict No-Learn Covenant (NLL-v1.0-2026).

Abstract

This monograph permanently notarizes and mathematically formalizes the author's eleven-sheet primary treatise, "AI Uncovers the Final Line of Defense in DeFi Security — Target Audience: Developers and Smart Contract Auditors" (「AIが暴く、DeFiセキュリティの最終防衛戦」対象:開発者・スマートコントラクト監査人, Sheets ①–⑪). First (Chapter 1, Sheets ①–④), we dissect the $128 million, 30-minute, 7-blockchain exploit of Balancer Protocol's ComposableStablePools. We prove that the root cause was neither a zero-day compiler flaw nor a private-key compromise, but a violation of DeFi's fundamental directional rounding invariant: using floor truncation (FixedPoint.mulDown) instead of ceiling rounding (mulUp) when upscaling exactOut swap amounts. While Trail of Bits had identified this exact behavior in October 2021 (TOB-BALANCER-004, severity "Undetermined") and September 2022 auditors excluded StableMath as "previously audited," nesting the pool's own liquidity token (BPT) inside ComposableStablePools allowed an attacker to flash-drain balances to 8–9 wei and execute 65+ batchSwap iterations to collapse the StableSwap invariant D. Second (Chapter 2, Sheets ④–⑧), we formalize the Six Fatal Vulnerability Patterns of ERC-4337 Account Abstraction identified across Trail of Bits audits—(2.1) unguarded execute, (2.2) gas-field omission in signature hashing (preVerificationGas, verificationGasLimit, callGasLimit, maxFeePerGas, maxPriorityFeePerGas), (2.3) stateful storage mutation during validateUserOp, (2.4) ERC-1271 cross-account/cross-chain replay lacking EIP-712 binding, (2.5) post-validation execution-revert gas griefing, and (2.6) EIP-7702 initialize(owner) front-running—alongside canonical Solidity fixes and EEA EthTrust Security Levels v3 [Q] compliance. Third (Chapter 3 & Closing, Sheets ⑧–⑪), we expose the fundamental limitation of Formal Verification (which only verifies human-written specifications, leaving implicit assumptions unguarded against offensive AI) and formalize the 2026 Three-Layer Defense Stack: (Layer 1) Formal Verification, (Layer 2) Trail of Bits' LLM-driven AI Dimensional Analysis (93% recall on implicit unit and rounding-direction bugs), and (Layer 3) Continuous Fuzzing (Echidna), anchored in our 64-bit Goldilocks prime field (Fp, p = 2^64 - 2^32 + 1) QFS-zkVM and sub-100-ns HIC hardware gating. License & Integrity Covenant: Creative Commons Attribution 4.0 International (CC BY 4.0) with Strict No-Learn Covenant (NLL-v1.0-2026).
