Remission of Widespread Erythema Annulare Centrifugum After Empiric Oral Fluconazole in a 54-Year-Old Woman: A Case Report
Erythema annulare centrifugum (EAC) is a rare reactive dermatosis that can be difficult to manage in persistent cases.A 54-year-old woman presented with pruritic, annular, and polycyclic plaques that subsequently extended to the neck, trunk, upper back, hands, lower legs, and feet.A skin biopsy supported the diagnosis of superficial EAC on clinicopathologic correlation. After limited improvement with topical therapies and oral prednisolone, she received four weekly doses of empiric oral fluconazole 150 mg without adverse effects. The eruption improved, and approximately five months after treatment discontinuation, she remained clear apart from residual post-inflammatory hyperpigmentation. The temporal association suggests a possible treatment response; however, the absence of pretreatment mycologic testing and the possibility of spontaneous remission limit causal interpretation.
