Reproductive-age status and intracranial hypertension in women with cerebral venous thrombosis: a retrospective cohort study

BACKGROUND: Cerebral venous thrombosis (CVT) predominantly affects reproductive-age women, but the relationship between reproductive-age status and intracranial hypertension (IH) phenotype remains unclear. METHODS: We performed a retrospective cohort study of consecutive female patients with first-ever acute or subacute CVT treated at a tertiary stroke center between September 2014 and March 2023. Patients were stratified into reproductive-age (18-45 years) and non-reproductive-age (>45 years) groups. Clinical characteristics, imaging findings, and 12-month outcomes were compared between groups. RESULTS: = 0.007) were independently associated with IH. Although overall functional outcomes at follow-up were similar, residual visual impairment was significantly more frequent in reproductive-age women with IH. CONCLUSIONS: Among women with CVT, reproductive-age status is associated with a greater likelihood of IH. In reproductive-age women, superior sagittal sinus and left lateral sinus system involvement were independently associated with IH, although the laterality-specific finding requires further validation.
