Echocardiographic Assessment and Clinical Applications of the Left Atrial Stiffness Index in Cardiovascular Disease
The left atrial stiffness index is an emerging echocardiographic marker that integrates pressure-related burden and atrial reservoir deformation to provide a composite measure of left atrial pressure-deformation coupling. This review summarizes the pathophysiological basis of left atrial stiffness, the methodological principles underlying echocardiographic left atrial stiffness index assessment, and the current evidence supporting its clinical applications across cardiovascular diseases. The strongest evidence is available in heart failure and atrial fibrillation, in which increased left atrial stiffness index has been associated with adverse hemodynamics, exercise intolerance, rhythm recurrence, and adverse clinical outcomes. In hypertension, cardiomyopathies, valvular heart disease, and congenital or aortic disorders, available data suggest potential clinical relevance, although the evidence remains more heterogeneous and less mature; by contrast, evidence in ischemic, inflammatory, and stroke-related settings is still preliminary. Despite its physiological appeal, broader clinical adoption remains limited by methodological heterogeneity, variable thresholds, limited invasive validation, and uncertain incremental value beyond its component measures. At present, left atrial stiffness index is best positioned as an adjunctive marker for phenotyping, hemodynamic assessment, and risk stratification, particularly in heart failure and atrial fibrillation, pending further methodological harmonization and prospective disease-specific validation.
