Cross-border preparedness, health worker protection, and institutional response in the 2026 Bundibugyo virus disease outbreak

This Perspective presents the author’s own analytical position on the international response to the 2026 Bundibugyo virus disease (BVD) outbreak in the Democratic Republic of the Congo (DRC) and Uganda, rather than a systematic assessment of the evidence. On 17 May 2026, WHO declared the outbreak a Public Health Emergency of International Concern (PHEIC), the second-highest alert level under the International Health Regulations (IHR) as amended in 2024. Three interlinked themes are examined: cross-border preparedness under the amended IHR and the WHO Pandemic Agreement (adopted 2025, not yet in force); frontline health worker protection, illustrated by four healthcare-worker deaths within four days of the initial alert; and institutional response and research investment, illustrated by the continued absence of a licensed, Bundibugyo-specific vaccine or therapeutic and by disparities in access to advanced care. Genuine early action has coexisted with weak cross-border screening coordination, gaps in healthcare worker protection, and limited community engagement, illustrated by the burning of an Ebola treatment centre in Rwampara; these gaps appear, on the evidence available, to reflect governance weaknesses, financing shortfalls, and insecurity rather than any single cause. The Perspective closes with recommendations on cross-border surveillance protocols, healthcare worker protection, community engagement, and equitable neglected-pathogen research investment, for policymakers, Africa CDC, WHO, and practitioners.
