Chinese consensus on the management of pancreatic portal hypertension

Pancreatic portal hypertension (PPH) is a distinct type of portal hypertension caused by various pancreatic diseases or surgery. In recent years, the detection rate of PPH has steadily increased with the widespread application of advanced imaging techniques and endoscopy. Its clinical manifestations are often insidious, compromising early identification. Notably, some complications, such as splenomegaly and upper gastrointestinal bleeding, frequently occur at advanced stage of PPH. Although the concept of PPH has been recognized in existing guidelines and/or consensus on the management of pancreatic diseases or portal hypertension, its management strategy has not been comprehensively summarized. Accordingly, the Chinese Society of Gastroenterology of the Chinese Medical Association invited a panel of skilled experts to participate in the Delphi consensus by systematically reviewing and discussing the available evidence on the causes, risk factors, clinical manifestations, diagnosis, therapy, and prognostic assessment of PPH. After three rounds of questionnaire surveys and votes, 19 consensus statements were formulated to provide the clinical guidance on the diagnosis and management of PPH.
