Optimizing the diagnosis of urosepsis: justification for the inclusion of IL-6 gene expression in the diagnostic algorithm

The original article presents the results of a study of the diagnostic significance of interleukin-6 gene expression in urosepsis in patients with inflammatory kidney diseases. Objective. To substantiate the inclusion of IL-6 gene expression determination in the diagnostic algorithm for patients with inflammatory kidney diseases as a molecular genetic marker of inflammation generalization. Material and methods. The study included 40 patients and 10 healthy volunteers, divided into three groups. Group 1 consisted of patients with a complicated course of inflammatory kidney diseases and signs of a systemic inflammatory reaction (n=20). Group 2 consisted of patients with an uncomplicated course of the inflammatory process without signs of a systemic inflammatory reaction after surgical treatment of urolithiasis (n=20). The control group included 10 healthy volunteers without comorbid pathology. A prospective analysis of the indicators of the systemic inflammatory response was carried out, which included an assessment of the patients’ condition on the SOFA scale, determination of the level of procalcitonin (PCT), C-reactive protein (CRP), leukocytes, leukocyte intoxication index (LII), and IL-6 gene expression. Results. The findings revealed statistically significant differences in IL-6 gene expression levels between the groups, with values being 3.6 times higher in Group I patients. There was a significant strong direct relationship between the IL-6 gene expression level and the SOFA scale in group I patients (r=0.79) and the inflammatory response in patients of both groups (leukocyte count r=0.80, LII r=0.82, PCT r=0.85, CRP r=0.78). Conclusion. The study results demonstrate a strong association between IL-6 gene expression and traditional inflammatory markers, confirming that IL-6 gene expression level can serve as an indicator of inflammatory process severity.
