Culturally Sensitive End-of-Life Care for Persons With Dementia: A Global Scoping Review and Conceptual Framework

INTRODUCTION: Cultural beliefs, care preferences, and social and health care contexts influence end-of-life care for persons with dementia. This scoping review synthesized global literature on cultural sensitivity in dementia care and developed a conceptual framework to guide research and practice. METHODS: Using Arksey and O'Malley's five-stage framework and PRISMA reporting, databases were searched from 2000 to 2023. Of 2104 records screened, 45 articles met inclusion criteria after quality appraisal. Data were synthesized using numerical summary and thematic analysis. RESULTS: Five themes emerged: cross-cultural decision-making differences, racial and ethnic disparities, patient and family experiences, spiritual and religious aspects, and health care system influences. Cultural factors, caregiver roles, communication, and social determinants shaped care. DISCUSSION: Culturally sensitive end-of-life care integrates sociocultural, ethnic, spiritual, and personal values through shared decision-making, caregiver engagement, and transition-focused education across care settings. CONCLUSION: Cultural sensitivity is central to high-quality dementia care, guiding research, intervention development, and clinical practice.
