Femoral Vein Trauma Associated with Medial Thigh Lift Plastic Surgery
Introduction We present a case report of common femoral vein (CFV) trauma occurring during a medial thigh lift (MTL) combined with liposuction, treated by thrombectomy of the superficial femoral vein and implantation of a great saphenous vein interpositional graft into the CFV via end-to-end anastomosis. Case report A 42-year-old female underwent medial thigh lift (MTL) complicated by massive hemorrhage near the right groin. Emergent situation was treated with three stitches, which were placed without visualisation of the bleeding vessel. On the first postoperative day, massive, painful swelling of the right lower extremity occurred. The patient underwent a computed tomography angiography (CTA) of the lower extremities. CTA revealed an absence of flow CFV and superficial femoral vein (SFV) indicated acute thrombosis. The patient was indicated for an open repair. Under general anaesthesia and full heparinization, the CFV vein was opened, and thrombectomy of the SFV was performed. The great saphenous vein was harvested, and a small interpositional graft was implanted into the CFV via end-to-end anastomosis. Patient was discharged on the 10th postoperative day on a daily dose of 100 mg of acetylsalicylic acid. Conclusion CFV trauma is rare, and there is limited evidence regarding its management, treatment, and outcomes. Endovascular techniques are not always feasible due to anatomical changes caused by trauma. Repairing the damaged common femoral vein with a great saphenous vein graft restored normal blood flow.
