Non-aneurysmal Subarachnoid Hemorrhage Following Routine Dental Extraction: A Case Report

We present the case of a 65-year-old man with multiple chronic comorbidities who developed severe headache and confusion three days after a routine dental extraction and was found to have a nonaneurysmal subarachnoid hemorrhage (SAH).On initial imaging, he had a diffuse SAH, and the ventricles were dilated, but no aneurysms or vascular malformations were identified on angiography. This was managed with the insertion of a right frontal external ventricular drain (EVD) to divert cerebrospinal fluid (CSF), with close monitoring in the intensive care unit (ICU).The patient experienced gradual neurological and clinical improvement during a 12-day stay at the hospital, and fluid balance, electrolytes, pain management, and complications, including pneumocephalus, were carefully managed. The EVD was successfully weaned and removed once neurological status remained stable and repeat imaging was reassuring. The patient was thereafter transferred to the neurosurgical ward for regular monitoring and repeat imaging, followed by discharge with outpatient follow-up.This case highlights the importance of multidisciplinary care and close monitoring in managing non-aneurysmal SAH with hydrocephalus in patients with complex medical histories.
