Clarifying statistical comparisons and procedural change in the TAPP learning curve

To clarify the interpretation of chronological changes reported in a recent study of laparoscopic transabdominal preperitoneal (TAPP) inguinal hernia repair. We reconstructed selected contingency tables using only aggregate counts reported in the published article. These calculations do not reproduce the original patient-level analysis. We also considered how concurrent changes in operative practice may contribute to the interpretation of decreasing operative time. For complications occurring in 8, 4, and 2 of 42 patients across the three phases, the published counts yield an overall Pearson chi-square p = 0.105 and a two-sided Fisher–Freeman–Halton exact p = 0.135, whereas the article reports p = 0.048. An ordered trend remains detectable ( p = 0.038), consistent with a temporal decline in complications. Operative time also decreased substantially across phases. Concurrent reduction in mesh fixation may have contributed to, but does not negate, this improvement. The published aggregate data warrant clarification of the statistical procedure underlying the reported overall comparison. Distinguishing overall group differences from ordered temporal trends and considering concurrent procedural adaptation may refine interpretation of the observed learning process.
