The Bio-Longevity Dental Score (BLDS): A Proposed Framework for Integrating Standardized Oral Health Assessment into Systemic and Longevity Medicine

The association between oral health and systemic longevity is increasingly well documented. In daily clinical practice, however, oral status is rarely translated into a single, communicable metric that can be incorporated into longevity-oriented care. To address this gap, we present the Bio-Longevity Dental Score (BLDS), a weighted, age-adjusted framework that quantifies the oral contribution to modifiable inflammatory and structural factors on a 0-100 scale. Informed by recurring clinical questions in preventive and biologically oriented dental practice, the BLDS is designed to make complex oral findings more transparent and easier to discuss across disciplines and with patients. This paper (version 1.0) sets out the model architecture – the subtractive formula, five categories and their fixed hierarchy, the age-weighting principle, and five interpretive bands – as a citable conceptual framework. Consistent with a controlled-instrument approach, this paper does not present the quantitative calibration, including category weights, age-band coefficients, band thresholds, the per-finding matrix, or the detailed operational protocol. These elements are maintained and version-controlled by the International Society of Bio-Longevity Dentistry® (ISBLD®) and released in successive layers through licensed training to support standardized application and inter-rater reliability. The BLDS does not claim to predict individual lifespan. Rather, it quantifies an oral contribution to factors that current evidence associates with accelerated biological aging. Subject to prospective validation, independent evaluation, and transparent version control, the BLDS aims to develop into a standardized assessment framework for oral contributors to longevity-relevant inflammatory and structural factors. We invite researchers and clinicians to examine, test, and contribute to validation of the framework.
