Parental attitude towards chiropractic intervention before and after treatment in infants with an arthrogenic torticollis: an explorative qualitative study

Abstract Background Torticollis is a common condition in infants, affecting up to 16% of newborns, most often presenting as congenital muscular torticollis. In some cases, however, it has an arthrogenic origin, referred to as arthrogenic torticollis (AT), prompting parents to seek care, including chiropractic treatment. Given that chiropractic treatment of infants may be unfamiliar to parents, it can evoke ambivalent responses among parents. However, little is known about how parental attitudes toward this approach evolve over time. This study aimed to explore changes in parental attitudes toward chiropractic treatment for infants with AT and to identify factors shaping their experiences and expectations. Methods In this explorative qualitative study, families with infants with a suspected diagnosis of AT who were referred to chiropractic care were recruited. Semi-structured interviews were conducted at baseline ( n = 12), and at follow-up ( n = 7). Data were analysed using a deductive-inductive qualitative analysis following Kuckartz’s approach, with coding in MAXQDA 24 and reporting guided by the COREQ checklist. Results Four main themes were identified through content analysis, the main themes were: Everyday life, Perception of infant’s health status, Prior knowledge, Attitude towards chiropractic treatment. Parents initially experienced uncertainty and emotional strain due to unfamiliarity with chiropractic care and witnessing manual treatment of their infant. Over time, as they became familiar with the process and observed improvements in their infant’s condition, their confidence, trust in professionals, and acceptance of the treatment increased. Key factors influencing attitudes included clear communication, observable health benefits, and prior experience with therapeutic interventions for their infant. Conclusion Attitudes of parents towards chiropractic care were shown to evolve from initial uncertainty to greater confidence and acceptance as they observed improvements in their infant and gained trust in the process. These findings highlight the importance of clear communication and guidance to facilitate positive engagement by families with unfamiliar treatments, thereby reducing stress during their infant’s care.
