Titanium-Zirconium (Ti-Zr) Narrow-Diameter Implants in Posterior Teeth: Current Evidence, Clinical Indications and Long-Term Outcomes

Titanium-zirconium (Ti-Zr) alloy narrow-diameter implants have emerged as an important development in contemporary implant dentistry, particularly for situations in which conventional-diameter implants cannot be accommodated without extensive bone augmentation. Their increased mechanical strength compared with commercially pure titanium permits the use of reduced implant diameters while maintaining favourable biological characteristics. Although narrow-diameter implants were traditionally associated with anterior tooth replacement, accumulating clinical evidence supports their application in selected posterior sites, including premolar and molar regions. This review examines the material characteristics, biomechanical rationale, clinical indications, surgical and prosthetic considerations, survival, marginal bone stability, mechanical complications, and long-term outcomes of Ti-Zr narrow-diameter implants in posterior teeth. Current evidence indicates high short- and medium-term survival, generally comparable with conventional-diameter titanium implants, with satisfactory marginal bone preservation. Recent evidence extending to five years is encouraging; however, robust posterior-specific long-term randomized clinical trials remain limited. Appropriate case selection, adequate bone quality and volume, controlled occlusal loading, prosthetic design, meticulous maintenance, and management of patient-related risk factors remain essential for predictable outcomes. Ti-Zr narrow-diameter implants should therefore be regarded as a valuable treatment option rather than a universal substitute for standard-diameter implants.
