Management of Mandibular Angle Fracture: Case Report

Introduction: Mandibular angle fractures frequently occur in facial trauma and can be treated openly (open reduction and internal fixation) or closedly (maxillomandibular fixation/closed reduction) depending on the fracture pattern, dental condition and clinical circumstances. The closed approach is still relevant for favorable fractures and patients without significant comorbidities. The present case report aims to describe and evaluate the management of a right mandibular angle fracture. Treatment was performed using closed reduction.Case: A 39-year-old male patient presented with pain when chewing in their lower right teeth. Approximately 1 month prior to hospitalization, the patient had fallen from a roof from a height of ∼3 m, striking the cement floor with their right face. The patient was then taken to the community health center for wound care. The patient was subsequently referred to Sinjay Hospital. Approximately 3 days after treatment in the emergency room, the patient complained of loose lower teeth and was referred for a panoramic radiograph and computed tomography scan. A closed reduction using an Erich arch bar and elastic intermaxillary fixation (IMF) for 8 weeks was selected as the treatment. Functional disorders caused by bone and dentoalveolar damage should be treated with anatomical reduction based on good occlusion. Non-surgical reduction has the advantages of no extraoral scarring and a lower risk of postoperative complications.Conclusion: Closed reduction with IMF is a viable option for favorable angle fractures in selected patients. The IMF method was chosen because these fractures rarely cause occlusion disturbances and because access to the fracture is difficult, conventionally managed with closed treatment.
