Ayurvedic Management of Psoriasis (Eka Kustha): An Approach Through Sodhana and Samana Therapy

Psoriasis is a chronic, immune-mediated inflammatory skin disease characterised by well-defined erythematous, scaly plaques with a recurrent and remitting course. It may significantly affect the physical, psychological, and social well-being of patients. In Ayurveda, its clinical features closely resemble those of Ekakushtha, a subtype of Kshudra Kushtha. The present case study evaluated the efficacy of an integrated Ayurvedic approach in the management of psoriasis. A 40-year-old male presented with dry, elevated, scaly and inflamed lesions over the hand and leg for two years, with recent aggravation for two months. Mild itching and absence of sweating over the affected areas were also reported. Examination revealed Matsyashakalopama, Mandala, Kandu, Bahalatva, Aswedana and Rukshata, with a positive Auspitz sign. Management included internal application of Snehapana. Following 15 days of treatment, marked clinical improvement was observed, with reduction of the PASI score from 4.5 to 1.02. The case indicates that a comprehensive Ayurvedic treatment protocol incorporating internal and external application of Snehan karma with Mahatikta ghrita followed by Shodhana karma may be useful in reducing disease severity and improving clinical features in psoriasis in day-to-day clinical practice and it is administered in out-patient and it is managed simply by out-patient department.
