Household-Reported Chronic Health Conditions, Behavioral Indicators, Maternal Care Utilization, and Ayushman Card Possession Among 9,329 Surveyed Households in Selected Communities of Madhya Pradesh, India: A Cross-Sectional Baseline Assessment.
Abstract Background: Community-based baseline assessments can identify local health needs and inform primary healthcare planning. This study assessed household-reported chronic conditions, behavioral indicators, maternal care, under-five vaccination channels, and Ayushman card possession in selected communities across six districts of Madhya Pradesh, India. Methods: A cross-sectional community baseline survey was conducted from 27 August to 5 September 2026 in selected communities of Bhopal, Gwalior, Indore, Jabalpur, Khargone, and Sagar. Accessible households in participating clusters were approached using convenience sampling with enumeration of participating households by local community tuition teachers. Data were collected using mobile Google Forms across 29 household-level variables. Descriptive proportions, recorded patient tallies, and exploratory unadjusted bivariate comparisons were calculated. Associations were assessed using Pearson's chi-square tests with Benjamini-Hochberg false discovery rate adjustment. No multivariable modeling or individual-level denominators were used. Results: Among 9,329 households, 2,576 (27.61%, 95% CI: 26.71%-28.52%) reported at least one of five measured noncommunicable diseases: diabetes, hypertension, cardiovascular disease, stroke, or cancer. Multimorbidity was reported in 852 households (9.13%). Household-reported hypertension was 1,596 (17.11%), diabetes 1,283 (13.75%), cardiovascular disease 389 (4.17%), stroke 308 (3.30%), and cancer 104 (1.11%). Bedridden status was reported in 372 households (3.99%). Tobacco use was reported in 4,153 households (44.52%), alcohol use in 3,915 (41.97%), and both in 2,816 (30.19%). Dual tobacco and alcohol use was associated with cardiovascular disease (OR 2.04, 95% CI: 1.66-2.50), stroke (OR 1.82, 95% CI: 1.45-2.29), and bedridden status (OR 1.79, 95% CI: 1.45-2.21). Among 2,070 households reporting a pregnant woman, 1,489 (71.93%) reported MCP card possession and 1,450 (70.05%) reported regular ANC attendance. Among 4,027 households with under-five children, 6,765 children were recorded; Anganwadi centres were the reported vaccination channel for 83.34%. Ayushman card possession was reported by 6,027 households (64.60%). In Chogna, Sagar district, 49 of 55 surveyed households (89.09%) reported stroke; Sagar stroke reporting was 7.01% including Chogna and 2.85% after excluding Chogna in sensitivity analysis. Conclusions: The surveyed communities showed substantial household-reported chronic disease and behavioral risk burdens, alongside gaps in regular maternal care and Ayushman card possession. Non-probability sampling, household-level reporting, and lack of clinical validation limit generalization to statewide or population-level prevalence. Findings may nevertheless help identify priorities for community screening, primary care linkage, and social scheme navigation.
