Beyond Symbolic Internationalization: Hidden Supervisory Burdens in International Clinical Training Programs

International clinical training in radiopharmaceutical production requires supervision within time-sensitive workflows involving radiation safety, quality control, and multidisciplinary coordination. This practice-informed perspective examines how a mismatch between institutional training commitments and frontline supervisory capacity may create hidden operational burdens. It discusses trainee preparedness, staged participation, protected supervisory time, clear responsibility allocation, and escalation pathways as elements of sustainable program design. Drawing on experience from a single hospital-based environment, it offers a conceptual framework for supporting both trainees and supervisors while maintaining clinical operations. This preprint has not been peer reviewed. It reports no original participant dataset, quantified supervisory workload, or measured intervention outcomes. The proposed relationships and governance responses have not been empirically validated.
