Outcomes of Eyes with Concurrent Suprachoroidal Hemorrhage and Retinal Detachment: A Multicenter Study

PURPOSE: To characterize eyes with concurrent suprachoroidal hemorrhage and retinal detachment (SCH-RD). METHODS: Retrospective study of eyes with SCH-RD from two institutions between 1/1/2013 and 12/31/2022. The primary outcome was the 6-month change in visual acuity (VA). RESULTS: Forty-four eyes of 44 patients (mean age 64.5 years, 40.9% female) were included. The RD was rhegmatogenous in 31 (70.5%) cases, exudative in 11 (25.0%) cases, and tractional in 2 (4.5%) cases. Proliferative vitreoretinopathy (PVR) was present in 22 (50.0%) cases. The mean (SD) presenting VA was 2.50 (0.46) logMAR (hand motion to light perception). Thirty-one (70.5%) cases underwent attempted surgical repair of RD at a median (IQR) 13.0 (6.0, 23.0) days after RD diagnosis. Of these 31 cases, 11 (35.5%) cases achieved single surgery success, defined as retinal attachment at 6 months with one surgery. At 6 months after presentation, the mean (SD) VA improved by 0.26 (0.78) logMAR [13.0 letters], with 19 (43.2%) cases achieving ≥ 3-line VA improvement and 4 (9.1%) cases achieving 20/200 VA or better. In multivariable analysis, greater VA improvement was associated with worse presenting VA and the absence of PVR. Twenty-seven (61.4%) cases achieved retinal attachment at 6 months. In multivariable analysis, greater odds of retinal attachment were associated with undergoing multiple surgeries for RD repair and the absence of PVR. CONCLUSION: Eyes with SCH-RD have guarded visual prognoses, especially when PVR is present. Undergoing multiple surgeries for RD repair and the absence of PVR were associated with greater odds of achieving retinal attachment.
