An audit of evidence-based psychological interventions offered to, and accepted by, patients experiencing psychosis for whom antipsychotics have not worked

Background Implementation rates of psychological interventions among individuals experiencing psychosis for whom antipsychotics have not worked (P-ANW) are poor. This audit aimed to determine the proportion of individuals experiencing P-ANW under the care of a Clozapine Clinic who were offered and accepted psychological intervention against the National Institute for Health and Care Excellence standard and if this varied by demographic characteristics.Methods A National Health Service Trust’s Research Database storing anonymised clinical data was scanned for structured data, clinical contacts and written correspondence from 1 January 2019 to the most recent update on 14 October 2024.Results Among 62 individuals experiencing P-ANW, 25.8% were offered NICE-recommended psychological intervention and 40.3% were offered any psychological intervention. Once offered, approximately two-thirds of the NICE-recommended group and three-quarters of the any intervention group accepted. Offer and acceptance were not associated with demographic characteristics, although descriptive trends suggest younger adults were more likely to be offered and accept intervention, and females were more likely to be offered.Discussion Rates of psychological intervention in individuals experiencing P-ANW remain low; however, once offered, individuals experiencing P-ANW are likely to accept. Services should consider how access is monitored and measures to increase implementation of NICE-recommended psychological intervention for individuals experiencing P-ANW.
