SUCCESSFUL EPITHELIALIZATION OF A CHRONIC PLANTAR ULCER AND DERMATITIS IN A DIABETIC PATIENT VIA ALTERNATIVE TOPICAL THERAPY: A CASE REPORT

Chronic diabetic neuropathic foot ulcers provide considerable treatment challenges due to compromised tissue repair mechanisms, ongoing inflammation and a high vulnerability to secondary infection. A 35-year-old female with poorly controlled diabetes, as demonstrated by fasting blood glucose of 156 mg/dl and increased postprandial blood sugar of 220 mg/dl presents with a refractory plantar ulcer lasting more than a year. Despite persistent hyperglycemia, which typically impedes wound healing by reducing angiogenesis and fibroblast proliferation, combining a targeted topical Biogenic Ulcer Care Ointment intervention with ongoing metformin therapy resulted in complete epithelialization within a four-week therapeutic window. The prescribed regimen included applying 5 g of Biogenic Ulcer Care Ointment three times per day, along with daily infrared thermotherapy. Topical botanicals often provide a concentrated matrix of bioactive phytochemicals, such as flavonoids, terpenoids, phenols, phytosterols and alkaloids, with strong bioactivity and antioxidant, anti-inflammatory and antibacterial effects. These components can accelerate granulation tissue formation, inhibit matrix metalloproteinases and increase keratinocyte migration. Simultaneously, localized infrared radiation induces cutaneous vasodilation while also increasing local tissue oxygenation and up regulating cellular metabolism. This combined treatment approach reduced local fibroblast activity and encouraged tissue regeneration, resulting in complete wound healing. Such immediate clinical outcomes not only reduce patient morbidity but also improve adherence to complex chronic care regimens.
