At its molecular core lies α-lactalbumin, which, upon calcium depletion, transitions into a flexible molten globule state and coordinates stoichiometrically with oleic acid (cis-9-octadecenoic acid) to self-assemble into HAMLET (Human Alpha-lactalbumin Made LEthal to Tumor cells) and its second-generation synthetic peptide derivative, Alpha1H. These complexes exert profound tumoricidal selectivity across more than 40 malignant human cell lines while leaving healthy, differentiated tissues intact. Rooted in a twenty-year trajectory of computational and clinical foresight, this paper establishes a comprehensive biophysical and pharmacological framework for Alpha1H/HAMLET-induced tumor-selective apoptosis. First, we delineate the molecular dynamics of lipid-membrane remodeling, where the complex exploits tumor-specific outer-leaflet phosphatidylserine exposure and elevated membrane fluidity to drive receptor-independent internalization. Within the cytoplasmic compartment, Alpha1H unleashes a coordinated multi-organelle assault ("the Hydra mechanism")—encompassing rapid ribosomal translational arrest, mitochondrial permeability transition pore (mPTP) collapse, unfolded protein response (UPR) hyperactivation, and non-canonical nuclear translocation leading to direct core-histone binding and irreversible chromatin condensation. Crucially, this cascade bypasses conventional p53 mutations and apoptosis-evading Bcl-2/caspase blocks. Second, synthesizing the Celtic conceptual paradigm of cyclical renewal with molecular pharmacology, we analyze the structural engineering that transformed fragile natural whey extracts into clinical-grade synthetic Alpha1H peptides. Third, we establish the synergistic immunological interplay between Alpha1H and Human Milk Oligosaccharides (HMOs), demonstrating how complex glycan decoys and microbiome-derived short-chain fatty acids (SCFAs) eliminate Single Points of Failure (SPOF) in host defense. Finally, evaluating robust Phase I/II clinical trial data in non-muscle invasive bladder cancer (NMIBC)—characterized by rapid tumor shedding and the downregulation of over 700 oncogenic transcripts—we propose a unified translational paradigm integrating precision genomics with broad-spectrum biopharmaceutical targeting. 要旨 (Japanese Abstract) 母乳は新生児に対する受動的な栄養供給媒体にとどまらず、数億年に及ぶ哺乳類の進化過程において研ぎ澄まされた多階層生体防御システムである。その中核を成すα-ラクトアルブミン(α-lactalbumin)がカルシウム解離により部分変性(溶融球状態:molten globule state)し、オレイン酸(cis-9-オクタデセン酸)と自己組織化結合することで形成される複合体「HAMLET(Human Alpha-lactalbumin Made LEthal to Tumor cells)」およびその第2世代合成ペプチド製剤「Alpha1H」は、40種類以上のヒト悪性腫瘍細胞を選択的に死滅させる一方、正常分化細胞には傷害を与えないという特異な薬理活性を示す。 本論文は、2004年の画像診断先見性から20年を経て結実した現代バイオ医療のパラダイム転換を基礎とし、Alpha1H/HAMLETの選択的腫瘍死滅機構を分子動態学的に解明する。第一に、腫瘍細胞膜のホスファチジルセリン露出および膜流動性異常を標的とする膜侵入動態、ならびにミトコンドリア脱分極・リボソーム翻訳停止・小胞体ストレス・ヒストン直接結合によるクロマチン断片化という多標的同時破壊(ヒドラのメタファー)を定式化し、p53変異やカスパーゼ耐性を無力化する機序を明らかにする。第二に、ケルト哲学の「死と再生の循環」に象徴される非炎症性アポトーシスの生物学的合理性と、天然抽出物からGMP準拠ペプチド製剤への工業的転換を論証する。第三に、母乳中に共存する200種以上の母乳オリゴ糖(HMO)によるグリコバイオロジー的デコイ機能・マイクロバイオーム免疫賦活との相乗効果を解明し、単一障害点(SPOF)を排除した分散型多層防御アーキテクチャを提示する。最後に、筋層非浸潤性膀胱がん(NMIBC)における第I/II相臨床試験の顕著な腫瘍シェディング効果および700以上の遺伝子正常化エビデンスを総括し、がんゲノム医療および免疫チェックポイント阻害剤との統合的製薬戦略を提唱する。
