A Rare Case of Arteria Peronea Magna (PAM): How We Handled It

Abstract Free osteocutaneous Fibula Flap is the most commonly used for the reconstruction of the mandible. The altered vasculature of the lower limb is the main reason for complications in flap harvesting. We present a rare case of Arteria Peronea Magna (APM) in a 57-year-old male who presented as a case of carcinoma of the oral cavity. The patient underwent composite resection of the mandible and sub-sites and was planned for reconstruction of the defect using a Free osteocutaneous Fibular Flap. Intraoperatively, it was found to have peroneal artery dominance. The procedure was abandoned, and the peroneal artery was re-anastomosed using a vein graft. Postoperatively, the recovery of the patient's leg was uncomplicated.
