A Novel Ultrasonographic Approach to Baastrup Disease (Kissing Spines Syndrome)

Baastrup disease (BD) is characterized by the close approximation of adjacent spinous processes, interspinous bursitis, and degenerative changes. Although BD is a well-known cause of chronic low back pain, it is often misdiagnosed due to limited clinical awareness and reliance on conventional imaging. While radiography, computed tomography, and magnetic resonance imaging remain central to the diagnosis of BD, ultrasound (US) can also identify interspinous bursitis, reduced interspinous space, and ligament involvement. Herein, we present two cases of BD with corresponding US images. This report aims to emphasize the diagnostic utility of US in detecting characteristic features of BD and to explore its potential role in therapeutic guidance. In this context, broader integration of US into clinical practice may improve the recognition and management of this frequently overlooked cause of low back pain.
