Living Well With Metastatic Breast Cancer: The Next Frontier for the Chronic Care Model?

Abstract Purpose of Review The overall survival of patients with metastatic breast cancer (MBC) is increasing, shifting it to a chronic condition for thousands of patients across the globe. We reviewed the literature to determine how to utilize the chronic care model for these patients. Recent Findings There are persistent gaps in the field of cancer survivorship for patients with MBC. The chronic care model can be applied to an MBC-specific patient population – including leveraging community resources, decision support tools like recent guidelines, and self-management support tools. Summary We have just begun to address the needs of cancer survivors, especially those with MBC. By implementing the chronic care model for these patients, we can provide more comprehensive and holistic care to this population, which has many unaddressed needs.
