A Novel Suture-Based Otoplasty Technique Combining Figure-of-Eight and Mattress Sutures for Prominent Ear Correction: A Retrospective Study of 89 Patients

BACKGROUND: Prominent ear is a common congenital deformity that can cause significant aesthetic and psychosocial concerns. This study introduces a novel suture-based otoplasty technique that combines figure-of-eight and mattress sutures to achieve natural-appearance aesthetic outcomes with minimal complications. METHODS: This retrospective study included 89 patients (116 ears) who underwent the modified technique between January 2023 and March 2025. The key innovation involves identifying the maximum tension point (MTP) and placing a figure-of-eight suture for tension distribution, combined with multipoint mattress sutures. Standardized photographic measurements of cranioauricular angles and distances were performed preoperatively and postoperatively. Patient satisfaction was assessed using a 3-point scale, and complications were systematically recorded. RESULTS: At the mean follow-up of 12.9±7.08 months, significant reductions were observed in cranioauricular angles (from 100.36 degrees±11.77 degrees to 43.49 degrees±9.01 degrees, P<0.001) and scaphoconchal angles (from 120.96 degrees±7.78 degrees to 87.76 degrees±9.56 degrees, P<0.001). Patient satisfaction was rated as satisfactory by 87.6% (78/89) of cases and fair by 12.4% (11/89), with no patient being dissatisfied. The minor complication rate was 9.0%, with no major adverse events reported. CONCLUSIONS: This technique represents a cartilage-sparing alternative with satisfactory aesthetic outcomes and low complication rates, making it a promising option for diverse age groups.
