Standing surgical extraction of a hypercementotic mandibular molar in a 14‐year‐old cob X gelding via lateral alveolar ostectomy
Abstract A 14‐year‐old cob X gelding was referred for further investigation of a complicated crown fracture of the left, first mandibular molar (Triadan 309) through pulp horns 3 and 4, with a further pulp horn defect (pulp number 2) and non‐vital pulp exposure. Radiographic changes consistent with a chronic periapical infection with associated reactive hypercementosis indicated a need for exodontia. Further fracture of the clinical crown and bulbous apical morphology precluded successful extraction per os. Surgical extraction of the tooth was performed via buccotomy and lateral alveolar ostectomy, under standing chemical restraint and locoregional anaesthesia. The apex was sectioned and elevated via the ostectomy site, and the remainder of the reserve crown repulsed into the oral cavity. The extraction site was packed with iodoform gauze and the buccotomy site closed in two layers. The horse recovered well from the procedure with excellent cosmesis and client satisfaction at 6‐month follow‐up.
