Case report – AML of left kidney with fatty embolus Involving the renal vein and IVC

A 53-year-old female was referred for radical nephrectomy on the background of known Angiomyolipoma (AML) of the left kidney. Serial CT scans demonstrated a 5.3cm fat-containing lesion in the left upper pole of her kidney and a fat density material within the left renal vein with level 1 inferior vena cava (IVC) involvement. Given the concern for pulmonary embolism an elective Robotically Assisted Radical Left Nephrectomy and IVC thrombectomy was performed. Intra-operative findings confirmed a mass on the left kidney and fatty thrombus within the IVC. Histopathological examination demonstrated 100% AML tumour and fatty thrombus with no evidence of malignancy.
