0071Correlation of Insulin Resistance with Cardiometabolic risks in Indian MAFLD patients: A Cross-Sectional study

Background Insulin Resistance (IR) is a common metabolic abnormality characterized by reduced sensitivity of peripheral tissues to insulin action, leading to disturbances in glucose homeostasis and lipid metabolism. It contributes significantly to pathogenesis of Type 2 Diabetes Mellitus (T2DM), cardiovascular complications and MAFLD. The Triglyceride-Glucose (TyG) index, derived from Fasting Blood Sugar (FBS) and fasting triglyceride measurements, is recognized as an acceptable surrogate marker of IR. Aim Correlating IR to cardiometabolic risks, dyslipidemia and T2DM in MAFLD patients. Methods This cross-sectional study included 84(40 M, 44 F) ultrasonographically diagnosed MAFLD patients. Lipid profile, FBS and HbA1c were obtained. TyG index and cardiometabolic indicators like Castelli Risk Index (CRI-I and II) and Triglyceride-HDL Ratio (THR) were calculated. Results The mean age of the study population was 47.8±11.9 years.The mean TyG index was 8.87±0.6, indicating high degree of IR in MAFLD patients. Significant associations were observed between the TyG index and total cholesterol (180±35.3mg/dL; p <0.001), triglycerides [median 141.5(IQR: 88.5- 208)mg/dL; p<0.001], and VLDL [median 28.3(IQR: 17.7-41.6)mg/dL; p<0.001]. The TyG index also showed significant associations with FBS [median 96.5(IQR:90-111)mg/dL; p<0.001] and HbA1c [median 5.6 (IQR:5.3-6.2)%; p<0.001], highlighting its close relationship with glycemic dysregulation .Furthermore, significant associations with cardiovascular risk markers, namely THR[median 3.2(IQR:2.3-5.3); p<0.001] and CRI-I [median 4.57 (IQR:3.8-5.5); p<0.001], suggest that the TyG index may serve as a useful indicator of cardiovascular risk in patients with MAFLD. Conclusions IR as assessed by TyG index is significantly associated with T2DM, dyslipidemia and increased cardiometabolic risks in Indian MAFLD patients. Is this an encore abstract? No
