Left Write Hook: proof of concept for a survivor-designed, and peer-led programme incorporating expressive writing and boxing to support mental health in adult survivors of child sexual abuse

Background: Existing approaches to supporting victim-survivors (herein referred to as ‘survivors’) of childhood sexual abuse (CSA) are resource-intensive and traditionally led by mental health professionals. Left Write Hook (LWH) is a novel survivor-designed, peer-led programme for survivors of CSA. By combining trauma-related expressive writing and non-contact, trauma-informed boxing, the programme promotes self-empowerment and supports survivors to rewrite their stories and reclaim their bodies.Objective: This study sought to demonstrate proof of concept for LWH as an intervention to enhance survivors’ mental health and wellbeing.Method: 17 women and gender diverse survivors completed an 8-week programme of weekly 2-hour sessions and completed both pre- and post-intervention assessments.Results: From pre- to post-intervention, we observed a significant decrease in self-reported symptoms of post-traumatic stress disorder, complex post-traumatic stress disorder, depression, anxiety and stress. We also observed a significant decrease in maladaptive post-traumatic cognitions and fragmentation of trauma memories, and a significant increase in emotional wellbeing and engagement in vigorous physical activity from pre- to post-intervention.Conclusion: Though evaluated in a small sample, these findings build on early pilot research (2020) and provide further support for LWH as a peer-led mental health intervention. LWH offers a multifaceted, empowering, and survivor-centred approach. This preliminary evidence illustrates the promise of LWH to support survivors.
