Gatekeepers in healthcare research: value, methodological challenges and practical recommendations

Background Gatekeepers play a central role in facilitating access to healthcare organisations, patients and professionals, making them integral to many healthcare research studies. While their contribution to recruitment and organisational access is well recognised, less attention has been given to the broader methodological implications of their involvement. As healthcare research increasingly emphasises transparency, reflexivity and rigour, greater consideration of gatekeepers is warranted. Aim To explore the role of gatekeepers in healthcare research, examine the opportunities and challenges associated with their involvement, and provide practical recommendations to support researchers in planning, conducting and reporting studies. Discussion This article examines the functions and value of gatekeepers in healthcare research. It considers their contribution to ethical practice, organisational access, participant engagement and study feasibility, alongside challenges including power dynamics, recruitment bias, organisational influence, researcher dependence and transparency. It concludes with practical recommendations for researchers before, during and after recruitment, emphasising careful planning, reflexive practice and transparent reporting. Conclusion Gatekeepers are essential collaborators whose influence extends beyond facilitating access to organisations, patients and professionals. Explicitly recognising and reporting their role can enhance transparency, support reflexive practice and strengthen the credibility of healthcare research. Implications for practice Researchers should engage proactively with gatekeepers while maintaining methodological independence and transparency. Greater attention to gatekeeper involvement can improve recruitment planning, support ethical conduct, and enhance the quality and trustworthiness of healthcare research.
