Abstract

A common paradox observed in exercise physiology and clinical nutrition is the premature cessation of fat loss—commonly referred to as a "weight loss plateau"—despite persistent caloric restriction and consistent physical activity. While frequently misattributed solely to patient non-compliance, this phenomenon is driven by complex, homeostatic biological adaptations designed to defend body mass. This perspective synthesizes the principal physiological, neuroendocrine, and behavioral drivers of weight loss resistance, including adaptive thermogenesis, non-exercise activity thermogenesis (NEAT) depression, body composition recomposition, chronic hypercortisolemia, sleep-mediated appetite dysregulation, dietary tracking inaccuracies, and the metabolic impact of the gut microbiome. Furthermore, evidence-based mitigation strategies, such as structured diet breaks, non-scale biometric assessments, and metabolic monitoring, are discussed to provide practitioners with an integrative framework for sustained metabolic health.
