Patient Fee Caps for Affordable Parkinson Assessment: Uncertainty, Subsidy Design, and Household Financial Protection

Abstract A low initial assessment charge does not guarantee an affordable clinical pathway when additional visits, interpretation, travel, or assistance generate uncertain payments. This article examines patient fee caps as one possible financing mechanism for accessible Parkinson assessment. Targeted narrative evidence on patient expenditure, digital participation, remote care, and economic evaluation informs a transparent analytical model. The model separates clinical resource costs, patient charges, external financing, and nonmedical cash expenses. Fully hypothetical scenarios demonstrate how capping a payment changes its distribution without automatically reducing the resources required for care. With three illustrative episode types, mean patient charges fall from 30 to 14 abstract cost units under a clinical-fee cap of 20, while the corresponding external-financing requirement rises from 25.5 to 41.5 units per episode. A separate cash-constraint calculation shows why protection depends on household resources and which expenses the cap covers. In the chosen example, including travel and assistance reduces the fraction of episodes within available household cash from 90% to 58.5% under the same clinical-fee cap. These are accounting and feasibility illustrations, not observed prices, uptake estimates, or measured project savings. The contribution is a framework for specifying the protected episode, payment timing, covered expenses, financing source, and clinical completion endpoint together. NeuralCipher's affordability objective is framed as reducing direct patient charges and associated household burdens, especially for people with limited financial resources.
