The Role of Medicare Administrative Contractors and Local Coverage Determinations in Dermatology Policy

Abstract Medicare Administrative Contractors (MACs) are central to the United States Medicare system, but their impact on dermatologic care is often overlooked. These private organizations process Medicare claims, enroll providers, and most importantly, develop Local Coverage Determinations (LCDs) that dictate which services are reimbursed in a region. We review the function of these entities and contrast them with private insurers. Opportunities to examine and participate in the policy-making process are presented to promote provider awareness. Finally, we summarized major differences in LCDs for Mohs micrographic surgery across various MACs to demonstrate variability. Our findings revealed geographic variability in reimbursement criteria that may impact patient access to care. Dermatologists should be informed about the coverage determination process and participate in policy development. Staying informed about evolving Medicare policies and their creation process is crucial to accessible, high-quality patient care and engagement in coverage policy.
