Comparative effectiveness of video-based and traditional education for home blood pressure monitoring in patients with chronic kidney disease: A pragmatic randomized controlled trial

Effective education for home blood pressure monitoring is vital for CKD management, but older adults face cognitive, sensory, and resource-related challenges. Video-based and traditional pamphlet education are used clinically, yet comparative effectiveness in stage 3–4 CKD remains unclear. To compare video-based versus nurse-guided pamphlet education for improving home blood pressure knowledge and skills in CKD patients. In this pragmatic RCT, 100 patients with stage 3–4 CKD were randomly assigned to video-based (n = 50) or pamphlet education with nurse guidance (n = 50), including interactive Q&A. Knowledge and skills were assessed at baseline and 3 months using validated instruments (Cronbach’s α = 0.95 for knowledge, 0.85 for skills). Ninety-six participants completed follow-up. Both groups showed higher knowledge and skills scores at the 3-month follow-up assessment. ANCOVA adjusting for baseline scores demonstrated no significant between-group differences in post-intervention knowledge outcomes (p = .884) or technical skills outcomes (p = .634). Adjusted post-intervention scores were comparable between groups. Video-based and pamphlet education achieved similar improvements, supporting flexible, resource-adapted strategies that respect older adults’ learning preferences and self-management needs. Nurses can select educational modalities based on patient digital literacy and institutional resources, with hybrid approaches optimizing engagement and learning outcomes.
