Integrated systems from small molecule modulators to next generation cell engineering: the emerging frontier in immuno oncology

Cancer immunotherapy has emerged as one of the most transformative approaches in oncology, offering new ways to harness the immune system to recognize and eliminate malignant cells. Over the years, a broad range of immunotherapeutic strategies have been developed, including small molecule immunomodulators, monoclonal antibodies, cancer vaccines, immune checkpoint inhibitors, and adoptive cell therapies. Each of these approaches acts through distinct mechanisms and targets different components of the immune system, yet all share the common goal of overcoming tumor immune evasion and improving anti-tumor responses. This review brings together these major classes of immunotherapy and discusses their molecular basis, therapeutic applications, and current clinical relevance across cancer types. Particular emphasis is placed on how these modalities are increasingly being viewed not as isolated strategies, but as interconnected and potentially complementary approaches that may work more effectively when rationally combined. In addition to highlighting major advances in the field, this review also considers key limitations, including resistance, toxicity, tumor heterogeneity, and the complexity of the tumor microenvironment. Overall, the growing integration of molecular, antibody-based, and cell-mediated therapies reflects an important shift in cancer treatment and points toward a more coordinated and effective future for immuno-oncology.
