Therapeutic Effect of 10-Minute K2EDTA Chelation in Moderate Band Keratopathy

Purpose: The therapeutic effect of ethylenediaminetetraacetic acid (EDTA) (define) chelation for band keratopathy depends on concentration, application time, and technique. We evaluated the efficacy of 10-minute dipotassium EDTA (K2EDTA) chelation with continuous rubbing in patients with moderate band keratopathy, and assessed disease recurrence after treatment.Methods: Moderate band keratopathy was defined as lesions extending from the peripheral to the central cornea, involving onethird to two-thirds of the corneal diameter, and affecting Bowman’s layer. Patients with recent ocular surgery within 3 months, severe dry eye, active keratitis, or an EDTA allergy were excluded. Five patients aged 18 years or older with moderate band keratopathy were treated with a 10-minute application of K2EDTA using an ophthalmic sponge with continuous rubbing. The resulting epithelial defects were managed with a dehydrated amniotic membrane and a therapeutic contact lens to promote healing.Results: Postoperative visual acuity was maintained or slightly improved. The corneal opacity observed before surgery was almost completely resolved, and epithelial healing was achieved within 1 to 2 weeks. At the 6-month follow-up, recurrence was observed in 1 of 5 patients (20%), which occurred at the site of the most severe lesion. No significant EDTA-related corneal damage or adverse effects were observed.Conclusions: In moderate band keratopathy, a 10-minute application of K2EDTA with mechanical rubbing, followed by placement of a dehydrated amniotic membrane and therapeutic contact lens effectively removed calcium deposits while preserving visual outcomes. Larger clinical studies with at least 1 year of follow-up are needed to define recurrence rates more accurately.
