Association of Preoperative Personality Traits with Six-Month Subjective Quality of Vision After Bilateral PanOptix Trifocal IOL Implantation: A Prospective Single-Center Study

Purpose: To investigate the association between preoperative personality traits and subjective quality of vision six months after bilateral trifocal intraocular lens (IOL) implantation. Methods: This prospective, single-center study included 66 patients undergoing bilateral implantation of the AcrySof IQ PanOptix TFNT00 trifocal IOL. Personality traits were assessed preoperatively using the Mini-International Personality Item Pool (Mini-IPIP). Six months after surgery, participants completed the Quality of Vision (QoV) questionnaire. Associations between the five personality dimensions and global QoV score, as well as individual visual symptoms, were evaluated. Results: Postoperative visual acuity was good at all distances (mean binocular uncorrected visual acuity: 0.04 ± 0.05 logMAR at distance, 0.06 ± 0.05 logMAR at intermediate, and 0.08 ± 0.05 logMAR at near). All five personality dimensions were significantly associated with global QoV scores (all pFDR < 0.05): extraversion (ρ = −0.343), agreeableness (ρ = −0.378), conscientiousness (ρ = −0.344), and openness to experience (ρ = −0.318) were associated with better subjective quality of vision, whereas neuroticism was associated with worse QoV (ρ = 0.428). Neuroticism showed the strongest correlation with starburst perception (ρ = 0.603, p < 0.001), while agreeableness demonstrated the strongest negative association with blurred vision (ρ = −0.524, p < 0.001). Halo, glare, starburst, hazy vision, blurred vision, and fluctuation in vision were consistently associated with multiple personality dimensions. Conclusions: Personality traits are significantly associated with patients’ subjective perception of visual quality following bilateral trifocal IOL implantation. Assessing personality before surgery may help identify patients at greater risk of postoperative dissatisfaction and guide more personalized counseling for those considering premium intraocular lenses.
