Customized Temporomandibular Joint and Mandibular Reconstruction Following Partial Mandibular Resection for Odontogenic Myxoma: A Case Report

ABSTRACT Background Odontogenic myxoma (OM) is a rare benign odontogenic neoplasm characterized by locally aggressive behaviour and a high recurrence potential. Extensive lesions involving the mandibular condyle often require radical resection, creating significant reconstructive challenges related to restoration of mandibular continuity, temporomandibular joint (TMJ) function and facial symmetry. Case Presentation A 21‐year‐old male presented with progressive enlargement of the left mandible and facial asymmetry. Computed tomography revealed an extensive multilocular lesion involving the mandibular body, ramus, coronoid process and condyle. Histopathology confirmed the diagnosis of OM. Owing to the extent of the lesion, a segmental mandibulectomy including the condyle was performed. Reconstruction was planned using virtual surgical planning (VSP) and patient‐specific CAD/CAM guides. Immediate reconstruction was achieved with a customized TMJ prosthesis composed of mandibular and cranial fossa components. Postoperative recovery was uneventful, with no major surgical complications. Mandibular continuity was successfully restored, with stable occlusion and satisfactory facial symmetry. Functional mandibular movements were achieved, although lateral excursions remained limited due to inherent constraints of the prosthetic reconstruction. Conclusion Customized TMJ and mandibular reconstruction associated with VSP provided a predictable solution for managing an extensive OM involving the condyle. Favourable functional and aesthetic outcomes were maintained during 18 months of follow‐up, with no evidence of recurrence or prosthetic complications.
